Purpose of Presentation: To address a critical element of patient care, safety-the need for health professionals to communicate effectively the provision of patient centered care. Background/Significance: According to the 1999 Institute of Medicine report To Error is Human, between 44,000 and 98,000 patients die each year due to medical errors.
