Fractures of the calcaneus frequently result from a high-energy axial loading mechanism. A predictable pattern of calcaneal fracture lines occurs after axial loading, produc-ing an anteromedial (sustentacular) fragment and a postero-lateral fragment.15 Operative management of intra-articular calcaneal fractures leads to improved patient outcomes in appropriately selected patients, particularly if the fracture involves the subtalar joint and if the posterior facet is ana-tomically reduced.5-7,19,22,23 The sustentacular fragment is critical to operative repair since it usually maintains its position in relation to the talus via the presence of strong interosseous talocalcaneal and deltoid ligaments and allows for accurate reduction of the other fracture fragments2 (Figures 1 and 2).
