Several large, randomized trials have established mycophenolate mofetil (MMF) as an effective immunosuppressant for kidney transplantation. Inhibition of inosine 5-monophospate dehydrogenase (IMPDH) by MMF appears to have pleiotropic effects on both immune and non-immune cells. In addition to B- and T-cell inhibition, in vitro and in vivo data suggest that MMF also has inhibitory effects on smooth muscle cells, endothelial cells, and myofibroblasts, findings that may be relevant in considering treatment of primary immune mediated kidney diseases.
