Changing paradigms in chronic condition care Depression and diabetes are two of the most common chronic conditions contributing to the global epidemic of chronic disease and illness. Diabetes affects nearly 3 % of the world’s population and is expected to at least double in prevalence in the next 20 years or so. Depression is increasingly common, and is expected to be the leading cause of burden of disease worldwide by 2020. There is also a growing interest in the co-existence of these conditions and how they are related, either aetiologically, or with respect to how clinical management of one may affect the other
