The concept that the composition of alveolar air may vary in space and time has occupied the at-tention of many investigators. Changes in ventila-tion and/or perfusion may alter the alveolar gas composition in normal lungs. The first published work on unequal ventilation in the lungs is usually attributed to Grehant in 1864. Recent studies (1-5) have involved measuring the concentration of a given gas in the expired air sampled at mouth level. Variation in gas concentration in the alveo-lar portion of expiration has been used as evidence for unequal intrapulmonary gas distribution.
