Review scope Included studies evaluated community-initiated or hospital-initiated (in or on discharge from acute care hospitals or emergency departments) CM interventions in persons ≥ 65 years of age; were done in Organisation for Economic Cooperation and Development countries; and reported unplanned hospital admis-sions. Studies reporting a mix of hospital admissions were included if unplanned admissions could be separately identified. CM, based on the UK King’s Fund definition, was considered a collab-orative process in the care of individual patients and included planning, coordinating, and reviewing care.
