This paper analyses the notion of responsibility with regard to the introduction of information systems. It sets out to give a brief account of the concept of responsibility and continues by describing in detail the attempt of the Irish League of Credit Unions to introduce a unified information system. In the paper we explore the reason for the failure of the project by using our theory of responsibility to identify possible subjects of responsibility. There are a multitude of potential subjects and no lack of responsibility ascriptions. However, the different responsibilities seem to have come into conflict and have been part of the reason for the failure of the project.
