We describe a case of retinopathy with swollen optic nerve head (ONH) in a hepatitis C patient treated with combination of interferon and ribavirin. A 33-years old man presented with sudden bilateral visual loss and headache. On examination, the Visual Acuity of right eye and left eye was hand movement and counting fingers respectively. Right fundus showed ONH swelling, healthy macula, with tortuous dilated vessels and small scattered, intra retinal haemorrhages. Retinopathy was only present in the right eye, however, poor vision in both eyes suggested neuro-visual involvement
