A 75 year old hypertensive male presented with history of sudden onset loss of consciousness 3 days prior to admission in our hospital. At the time of admission pulse rate was 112/min, BP was 190/ 110 mm of Hg, and random blood sugar was 126 mg%. The patient was found to be deeply comatose with no response or movement to deep painful stimuli. Both plantars were upgoing, pupils were unequal with sluggish reaction and skew deviation of the eyes was noted. Rest of the examination was unremarkable.
