Summary: Fenestrations of the cerebral arteries are un-common, and aneurysms in association with fenestrations are even more uncommon. We report the case of a saccular aneurysm associated with a posterior cerebral artery fen-estration in a child with subarachnoid hemorrhage. The patient was successfully treated with Guglielmi detachable coils. This case is unusual because of the patient’s age and the location of the fenestration. Fenestrations of the cerebral arteries are uncom-mon, with a reported angiographic incidence of 0.03–1 % (1–3) and a reported postmortem incidence ranging between 1.3 % (4) and 5.3 % (5).
